Step 1 - Virtual Consultation
- New Patient Forms & Photos
By sending your photos privately via
email and accessing our new patient paper work with the
click of a mouse, we can set up your personal consultation
with Dr. Miller quickly and easily. During your virtual
consultation done via telephone, you will speak directly
to Dr. Miller as he reviews your photos with you. You
and he will discuss your specific situation and all of
your questions and concerns will be addressed.
Step 1A - Downloading Forms
To begin the process, start by downloading the
New Patient Forms. You need to download the forms and fill in online if you are sending your package to us digitally via email or print all five forms under New Patient Forms, fill in by hand and mail with your package.
You will need to have Adobe Acrobat Reader installed on your computer to view these PDF forms.

Next see under Instructions and Photo Samples for Breast, Facial/Rhinoplasty and Abdominoplasty Patients for directions on how to take your digital photos to send us digitally via email or by postal mail.
Step 1C - Emailing your Forms and Photos or Sending by Postal Mail
If you are sending your forms and photos electronically via email they should be
submitted with a short description of your
desired procedure, expectations and concerns. You should attach your forms and digital photos to the email.
The subject line of your email should read:
Surgery Travel Program - Virtual Consultation
Email your packet to: info@millercosmeticsurgery.com
Your electronic package will be reviewed, and you will receive a response within 48-72 hours from your package arrival date as to a time and date for your Virtual Consultation with Dr. Miller.
If you are mailing your forms and photos they should be
submitted with a short typed description of your
desired procedure, expectations and concerns.

Your package will be reviewed, and you will receive a response within a week or two from your package arrival date as to a time and date for your Virtual Consultation with Dr. Miller.
Step 1D - Your Virtual Consultation with Dr. Miller
After your forms and photos are received, Dr. Miller's staff
will contact you within 48-72 hours if you sent an electronic package via email and up to a week or two if you sent your package via postal mail to schedule your Virtual
Consultation done via telephone. Please make sure you send us accurate and current phone numbers at which you can be reached and when is the best time. You will speak directly
to Dr. Miller as he reviews your photos with you. You
and he will discuss your specific situation and all of
your questions and concerns will be addressed. Note: This Virtual Consultation is considered a confirmed appointment with Dr. Miller and must be treated with the same respect as an in office exam. If you need to cancel or reschedule this appointment you must call our office at (866) 338-9444 48hrs prior to your Virtual Consultation to cancel or reschedule.

Breast Instructions and Photo Samples
1. Please take Photos in front of a plain background, example: a plain colored wall or plain colored sheet hung on a wall. Preferably a white, med blue, dark blue, or black sheet or wall.
2. Photos should be taken from just below the neck with shoulders visible to just above the belly button. Hands and arms should not be raised as this lifts the breasts to an unnatural position. Hands can rest on your hips with elbows bent naturally to the sides or rest arms naturally straight back. Be as relaxed as possible, for the best photo outcome.
3. Keep the camera angle straight forward, do not angle down or up as this will also create and unnatural position/view of the breasts.
4. You will need to take your photos without any garments on, for example, no bra or shirts.
5. You will need to take 1 forward shot, 2 quarter turn
shots and 2 side views of the breasts. Total of 5 images.

Facial/Rhinoplasty Instructions and Photo Samples
1. Please take Photos in front of a plain background, example: a plain colored wall or plain colored sheet hung on a wall. Preferably a white, med blue, dark blue, or black sheet or wall.
2. Photos should be taken from top/crown of head to just below the neck with collar bones visible. Facial features should be relaxed. No smiling or frowning, just a relaxed natural position. Keep jawline in a relaxed natural position, do not try to push jaw out or "pose", this will only obscure your natural jawline. Be as relaxed as possible, for the best photo outcome.
3. Keep the camera angle straight forward, do not angle down or up as this will also create and unnatural position/view of the facial features.
4. You will need to take 1 forward shot, 2 quarter turn
shots, 2 side views and 1 view with head tilted slightly
downward of the facial features. Total of 6 images.

Abdominoplasty Instructions and Photo Samples
1. Please take Photos in front of a plain background, example: a plain colored wall or plain colored sheet hung on a wall. Preferably a white, med blue, dark blue, or black sheet or wall.
2. Photos should be taken from just below the breast to below the belly button to right above above the pubic area. The whole abdomen area should be visible with no undergarments obscuring the area to be examined. Hands and arms should not be raised as this lifts the abdomen to an unnatural position. Do not "suck" in the abdomen as this also creates an unnatural position. Inhale and exhale out to completely relax your abdomen. Hands can rest on your hips with elbows bent naturally to the sides. Be as relaxed as possible, for the best photo outcome.
3. Keep the camera angle straight forward, do not angle down or up as this will also create and unnatural position/view of the abdomen area.
4. You will need to take your photos without any garments on, for example, no shirts covering the abdomen area.
5. You will need to take 1 forward shot, 2 quarter turn
shots and 1 side view of the abdomen area. Total of 4 images.
