The fees quoted for your procedure include Dr. Miller's fee, the use of the office surgery center, surgical supplies, nursing care and pre and post operative visits. Also included in your surgeon’s fee are Vitamedica Healing Supplements to enhance your surgical results and ease your recovery experience. The additional charge for anesthesia services must be paid separately from Dr. Miller's fee. The fees for any laboratory or preoperative testing are payable directly to the outside facility and are not included in the above fees.
To hold a date for surgery, a non-refundable deposit of $500 must be paid at the time of scheduling your surgery date. This fee is for the date chosen and is not transferable to other surgery dates. The deposit will be applied towards the cost of your surgery. Unless other arrangements have been approved, all fees for surgery are payable at your preoperative appointment,or three weeks prior to surgery. If your preoperative appointment is less than three weeks prior to surgery, payment must be made by cash, cashier's check or credit card, (MasterCard and Visa). You will be charged 20% of the total fees paid for any surgery that is cancelled after the preoperative appointment. If surgery is cancelled less than one working day prior to your surgery date you will be responsible for all operating room fees and 50% of all other fees paid.
